DPI Open Enrollment Information
Open Enrollment Period
o This is the open enrollment application period.
o
The recommended method of
applying for open enrollment is through the on-line
application process at
http://dpi.wi.gov/sms/psctoc.
o Paper applications must be submitted to the nonresident school district no earlier than April 30, 2012 and no later than 4:00 PM on February 24. A postmark does not meet this requirement. (PI 9410, also available in Spanish).
o The parent may request assignment to a specific school or program.
o The application form includes a request for transportation reimbursement assistance for low-income parents
o Any students attending a nonresident school district under a “Tuition Waiver Due to a Move” must apply for open enrollment in order to attend the nonresident district in the 2012-13 school year.

Over the years Lakewood has developed a strong tradition of solid core values with an eye on the future. All students deserve to be safe and respected at school. We hold this belief dearly and take regular steps to ensure this. Our students are challenged by a variety of activities both inside and outside of the school day. They deserve our best as we encourage them to do their very best.
Reading and literacy are critical in today's world. We have
worked tirelessly to develop
and implement a responsive educational program (RTI) the
involves early identification,
individualized interventions and progress monitoring. We are
actively using data to help drive these processes.
While we have
made great strides to improve our school, we realize we
cannot do it alone. A
commitment to our school community partnership is the key to
success. We appreciate your
support as we continue our quest to promote "Learning
Today", so our students can be "Leading Tomorrow".

Kindergarten Screening
If you know of a child living in the district that will be eligible for 4K or 5K kindergarten this fall, please have his/her parents call the district office at 262-877-2148 ext #162 as soon as possible to register their child. To be eligible to attend 4K or 5K Kindergarten in the fall, children must be four (4) or five (5) years old prior to September 1, 2012. The child’s birth certificate and a current immunization record will be required. Thank you for your assistance!
